Appointment Cards

Run Appointment Card forms to generate detailed appointment reminder cards for your patients. The information available for printing on the card, such as the most recent payment receipt, or a school / work excuse, depends on the style you select. You may also include your office name and address on the form as it prints, or have your logo pre-printed on special form paper for a more professional appearance. In addition, you may choose to email appointment cards to patients and / or responsible parties, rather than printing them.

Get Started

Set Up Electronic Appointment Card Format - Use the General and Appointment Card tabs of the Form Email Setup tool to set up the format and contents of appointment cards you email to patients. If you do not offer electronic appointment cards, you can ignore these settings. See "Set Up Form Email Options" for details.

Click to OpenEnable Electronic Correspondence - (Optional.) - Enable / disable the electronic correspondence option on the Patient and Responsible Party tabs of the patient folder to choose whether to send appointment cards to the patient's and/or responsible party's Email1 address, rather than printing them. If you enable both the patient and responsible party for electronic correspondence, an appointment card will be sent to each email address.

Run This Form

  1. Click to OpenOpen the Form - You can open this feature in multiple ways: Use the method that's most convenient for you.

  2. Click to OpenForm Settings - Select the options to use for choosing records and generating this form.

  3. Manage Record Selections & Output - After selecting the form options, click Ok to continue to your output choices. See "Manage Record Selection & Output" for details. If you have enabled electronic correspondence for the patient, responsible party, or both, the form will be emailed immediately to all enabled email addresses, and will not be printed. To print the appointment card instead, you can enable the Disable Electronic Submit option, above.

More Information

Appointment Length - The appointment length that prints on the appointment card is taken from the Appointment Card Length field of your Appointment Procedures table, not necessarily the length you have scheduled for the appointment. (See "Appointment Procedure Table" for details.)

School / Work Excuse Option - Although you can include an excuse slip any time you print a regular appointment card, you might want to print the slip at the conclusion of the appointment instead. See "School / Work Excuse Forms" for details.
