Run the Online Forms Questions report to obtain a copy of the questions that are in your online forms, as well as their corresponding mail-merge variables, making this report handy for inserting online form data into mail merge documents.
Download a completed form - You must have downloaded at least one completed online form before you can use this report. If you later add a new online form type, you must download a completed form of that type before it will be available for this report. See "Download & Link Online Form Results" for details.
ViewPoint main menu > Practice Reports > Table > Online Form Questions.
Select the form type - You may include All forms, or choose a specific type to print on the report.
Manage Record Selections & Output - After selecting the report options, click Ok to continue to your output choices. See "Manage Record Selection & Output" for details.
