Fill Out an Online Form

Launch Online Forms to complete a form for a new patient, or to complete a new or updated form for an existing patient.

Get Started

Add a Health History Link to Your Web Site - (Optional.) Use our Health History URL web page to add a hyperlink to your own website that patients can use to fill out new forms. Once you add the link, patients can then use that link to launch online forms from anywhere with an Internet connection. See "Set Up Health History URL Links" for details.

Install / Load Forms Viewer - (Optional.) You can load the online forms viewer on your workstation, then access online forms at any time without having to open your entire ViewPoint system. See "Install / Load Online Forms Viewer " for details.

How to Get Here

You can open this feature in multiple ways: Use the method that's most convenient for you.

Forms ViewerClick to Open (Requires the OII Forms Viewer. See "Install / Load Online Forms Viewer " for details.) Double-click or right-click the OII Forms Viewer icon in your system tray, and select the Launch New Form option. Enter your ViewPoint user name and password to continue.

Web Site Link - If you have added an online forms link to your website, you can use that link to launch forms. See "Set Up Health History URL Links" for details.

Paper Forms - In certain cases, you may need to complete forms on paper. To do this, simply print the .PDF file of the form, found on the Plug-Ins page of your ViewPoint main menu. You can also add a hyperlink to the .PDF file from your office web site, so patients can download and complete the paper form themselves.

Use This Feature

Choose ActionClick to Open Select whether you want to launch a new online form for a new (blank) patient, launch a new form for an existing patient, or update the data from an existing online form. Then, click Next to continue.

Choose Form Type & LanguageClick to Open  (Applies only when opened from Forms Viewer.) Choose the form and language to use, then click Launch to initiate the form.

If you select to launch a new form for an existing patient, or update a form, you then select the patient to work with. You can optionally select a responsible party as well. The form will then pre-populate with both the patient and responsible party biographical data. If you do not specify a responsible party, only the patient data is entered automatically.

Complete All Required FieldsClick to Open All required fields are indicated by an asterisk ( * ). You cannot submit the form until all these fields have been completed.

Submit FormClick to Open Click Submit at the bottom of the form to prepare the information for downloading to your database, and linking to a new or existing patient folder. See "Download & Link Online Form Results" for details.

If you attempt to submit a form without completing all required fields, a list of those fields will appear. You can then either complete the necessary fields, or click Clear to erase the form information and start over.

If you close the form window instead of submitting the form, all information will be lost, even if you have completed all required fields. If you opened the form with the Forms Viewer, you will see a message warning you that the form information will be lost if you continue. This warning does not appear if you open a form with a Web browser.

Delete Online Form Data - You can use Data Tool (135) - Delete Online Forms Answers from Patient to remove online form answers from a patient folder. See "Use Data Tools" for details.

Transfer Data Between Patients - If you accidentally assigned an online form to the wrong patient, you can transfer the information to another patient with Data Tool (134) - Transfer Online Forms Answers Between Two Patients. See "Use Data Tools" for details.

View & Update Online Forms From the Patient Folder - You can also access online form results and update data for individual patients directly from the patient folder. See "View & Update Online Form Results" for details.

Award Patient Reward Points - You can assign a number of points a patient earns toward your Patient Reward system when they complete an online form. Patients can earn points for completing the form the fist time, and for each time they update the form. The points are awarded when the form is downloaded and linked to the patient folder. See "Set Up Online Forms" for details.
