Online Forms provide a way for patients to fill out forms electronically, and a way for you to use that data in patient folders, merge letters and subgroups. Your online forms can be of any type (health history, office questionnaire, outpatient interview, etc.). Your system initially includes a new patient health history form. You can request additional customized forms for a nominal fee. In addition, you can include your own logo at the top of all online forms.
